      Use this application (use separate forms for each person) to submit an order for personalized book(s), photographs or stationery.  Fill in all of the appropriate sections--be as detailed as you can.  This can be very personal information, also know that all information will be kept confidential.  Nobody but me sees this form!
     ...And don’t forget to fill out the Order Form and send the application and order form in together, along with your payment.

SECTION TWO--Storybook

     The following information is needed to create a unique, personal story for your child.  It will include not only your child’s name, but also a friends’ name and some special things your child likes, plus the type of story line he or she likes (adventure, religious, etc), so please be as detailed as you can so that she can create a really fun story.

     If you provide any photographs of your child or their favorite things (or best friend), I will incorporate them into the alphabet book.  This is optional, but a great way to personalize the story.    Please make sure you identify your child (or item) on back of photograph.

     Whew!  You are done.  With all of the above information, I will now weave a fun short story or alphabet book appropriate to your child’s age and include a few illustrations (including the photos, if provided).  Then I will bind the book, and as mentioned before, will mail it to you within 8-10 weeks.  If there are any unforeseen delays, I will contact you by phone (or e-mail, if preferred).
